Who can use the
Student Health Center
All current students
enrolled and taking
classes at the
university and/or
affiliated campuses are
entitled to receive
services offered at the
Student Health Center.
Included are basic
laboratory tests,
unlimited visits,
immunizations,
educational programming
with confidential health
education or classes,
and other services. All
new customers are
required to submit a
completed initial
health/medical history
form and consent for
treatment. Proof
of required
immunizations is
requested. Employees may
be seen re: work-related
injuries/illnesses and
may be seen for limited
services i.e. Influenza
vaccine.

We are now
located at 333B
S. Stadium.
We are right
next to the
Football Stadium
and adjacent to
the new First
Care Clinic and
Sports Medicine
Clinic..

What are
the hours for
the Health
Center ?
Office Hours for
the APNs or
Nurse:
Monday - Friday,
8:00 am to 5:00
pm. Phone

Patients are
seen from 8:00
am to 4:00 pm.
Do I need
an appointment?
In order to
serve you
better, we
prefer you call
us at 972-2054
to make an
appointment with
the APN.
You may also
want to schedule
an appointment
with the RN for
immunizations
and/or health
education.
There is
sick-call daily
from 8 a.m. - 9
a.m. and 1 p.m.
- 2 p.m. for
acute illnesses.
What do I
do when the
Health Center is
?
You may choose
to go to any of
the following
facilities,
however, the
University does
not assume
responsibility
for payment of
Emergency Room
fees,
prescriptions,
or other outside
tests:

What do I
do in case of an
emergency with
myself or
another student?
In case of a
life-threatening
emergency (i.e.
heart attack,
stab wound,
choking, near
drowning) call
ASU Police at
"9-911" or
"911".
Also, the above
clinics and/or
hospitals
mentioned may be
able to handle
your emergency
and/or serious
health problem/
crisis. The
Student Health
Center is able
to handle minor
injuries and
illnesses.
What do I
have to pay for?
There are
certain fees for
some services,
and there are no
student tuition fees
collected to
fund the student
health center.
The center
receives
approximately
$1.00 for every
student at ASU,
so that is why
some services
are charged to
the student.

What about
confidentiality
at the Student
Health Center
Information
obtained and
recorded in your
medical chart is
kept strictly
confidential.
The medical
staff have
access to your
chart only.
Students are
protected under
patient/provider
confidentiality
provisions.
Medical
information may
be released only
with your
written
permission,
except in cases
specified by the
law.
What are
my rights and
responsibilities?
We keep the
students rights
and
responsibilities
posted in the
Student Health
Center so that
you can
understand your
role in your
health care.
You may review
these at
student/patient
"Rights and
Responsibilities".
Will I
ever be referred
to another
health care
provider?
Depending on
your specific
health care
needs, you may
be referred to a
specialist,
medical doctor,
or other health
care agency that
may be better
able to assist
your health care
problem or need.
For example, if
you are wanting a
Hepatitis B
vaccination and
your 20 years or
younger, you may
want to seek the
Health
Department
located on
Washington Ave.
in Jonesboro.

Do you see
employees at
ASU?
We do see
employees who
have a minor
injury and need
medical
assistance.
Also, we provide
services like
blood pressure
screening,
immunizations,
skin test, and
assist with
minor illnesses
when available.
The ASU student
is our first
priority with
all services
rendered.
